Bangiya Gramin Vikash Bank — transaction failed but amount debited from account

On 06.04.2020 i made three transaction for cash withdrawn from sbi, boro atm counter.in the second transaction of rs 5000, the transaction was not successful but the amount debited from my account. Its 5 days but the amount not back in my account. Please repay the amount as earliest. Account no ending with 371 and atm card no ending with 0568.

Sir/ Madam,
I'm Lakshmikanta Maity, an account holder of your bank at Gopiganj branch, Paschim Medinipur. My a/c no. is[protected]. A transaction of mine was failure on 16/12/2021.But ten thousand(10000.00) rupees was debited from my a/c. But still now it is not credited.
So, it is my humble request to you to solve my problem as soon as possible.
