Dear Sir,

Sub: Complain for Auto force closure of Policy no.ELP1885954

With reference to above, I the undersigned, Khimji Vejanand Chavda, would like to bring following facts to your kind notice :
1. I am account holder of PRIME BANK, Bharuch and bank staff does the work of Aviava Life Insurance. Mr. Mayur Dalal –Bank staff requested me take an insurance policy of Aviva so based on customer relationship I agreed for that.
2. Then after Mr. Mayur Dalal and Mr. Mahesh Parmar-Aviva Advisor came at my home for further procedure. At that time I cleared that I can pay premium maxm for 3 yrs and then after I want refund of money. Accordingly they took my details- Name, address, nominee name and signature on application form. No other conditions were discussed at that time.
3. I have paid premium of Rs.10000 upto 3 yrs and after that I visited at Aviva office Bharuch for refund but staff told that you will get money after 5 yrs.
4. On 1/8/12 I received a SMS on my cell that your policy is autoforce closed so I called on Aviva’s no.[protected] and gave my complain. I also informed to Prime Bank staff for this and he assured to inquire in the matter.
5. Again 14/8/12, I received SMS that refund cheque is dispatched. On 17/8/12 I recd a courier containing a letter and cheque of Rs.10000/- towards refund.
6. Policy was taken thru Prime Bank staff Mr.Dalal and he is also agreed for the facts that no condition for surrender value or autoforce closure were discussed. He also surprised that after investment of Rs.30000/- customer gets the refund value of Rs.10000/- from AVIVA even after 4½ yrs.

In view of above facts, I request to look into the following:
1. I have paid total premium of Rs.30000/- and after four years I am getting Rs.10000/- only by showing hidden conditions? Is this type of benefit given by Aviva Life Insurance?
2. It is possible that Aviva advisor has changed the plan for more commission. It is clearly seems that he has done cheating and breach of trust with customer.
3. No Aviva representative has talked to me about hidden conditions before autoforce closure and within 15 days send the refund cheque. I never expected this type of customer care approach from AVIVA.

Personally I feel that I have been cheated by agent Mr. Mahesh Parmar and its hard earned money by me and I want proper resolution of my complaint.

Its my humble request, atleast to refund me my total premium amount of Rs.30000/- as now I have no hope for more return on my investment. I have note deposited your refund cheque of Rs.10000/- in my account and it will be returned on demand by you.

Hope considering this as a serious matter you will take prompt action in this matter.
