Icici Lombard — Debited via credit card for a policy which i never signed up

ICICI bank issued a credit card againts my name which i never got. They issue one replacement card after some period without my information. It is also still with the bank undelivered. There was one policy (ICICI Policy) activated on it without my information. After cancelling this policy they deducted Rs. 10000 from my saving account via credit card. I never got the card. I never got any intimation for the actvation of the policy. I want all my money back as i have not signed up for any policy. I do not know how they got these weird ideas to disturb their customer. My parents are also got cheated from ICICI lombard. They were deducted money from their credit card for an insurance which they never asked for nor signed any with ICICI lombard. They did the same with my father and mother, when enquired and fought for months together, they cancelled my mothers but still they are debiting money from my fathers credit card.

Can anyone tell me some way to complaint against this fraudulent act of ICIC lombard?
