Hcl Tech — annexure k required from hcl

Hi,

I have just submitted form 13 with my current organization to get my pf transfer from hcl. Form 13 have been certified and submitted to pf department and the same accepted by field office. Now pf department required annexure k from hcl side to transfer my amount. Can some one please help me out on this.
