Itd — For pan card

I have applied for Pan No. for RAM DHANDRA s/o TULSI on dt.[protected] coopan no [protected] only for first time and still pending and showing below message but i have not applied any other pan no in past:

Income Tax Department (ITD) database shows that you may have been issued PAN in your name. ITD yet to inform status of your application.

I have made many calls on incometax helpline & NSDL helpline but they said every time wait for one weak now 4 month passed and his pan no still pending. bank has locked his account for pan. pls see if Income Tax Department (ITD) database showing that he may have been issued PAN in his name. then whose photo and signature is printed on it . if there is RAM CHANDRA's photo and sign then send it to him inhis given sddress (village and post bijua distt lakhimpur kheri up 262901 india) if not so then
I am requesting to you that please issue CHANDRA's pan no. soon
