In andhra bank, branch code 1198- medavakkam, tambaram, chennai-600100, i have joined in arogyadaan health insurance policy under andhra bank customers. The details of my health insurance is as follows:

The above details are from my lost year health card
The renewed card not yet received from andhra bank.
I have paid a sum of rs.17432/- towards revised premium for my health insurance on 10th june 2019.
Still since three months gone, i could not received the renewed card.
I have contacted my branch office medavakkam since 10 times and the reply is card not yet received.
I do not know where to complaint, whether, andhra bank, good health insurance or united india insurance c0. Ltd.
As i am old senior citizen at any time my health may give trouble for which i need the health insurance card immediately.
I will be helpfull if you could give a concrete reply to get my arogyadaan health insurance card,.
