Payment success for 2 times and the recheck form isn't filled nor may be printed

Address:Cipet ahmedabad plot no 630, industrial area phase 4 vatva ahmedabad 32445

I've paid recheck fee rs 100 and then I'm unable to see the submitted form or unable to print but the payment is succes and debited from my account, then I have tried for second time payment for the same rechecking rs 100 again I ve faced the same problem of submission and print issue but the payment is succes and debited from my account, so please solve my issue and provide me refund
