Cognizant — cognizant offer letter not received for interview on 07th apr 2018 at okkiam thoraipakkam chennai

I was interviewed on 20th jan 2018 at okkiam thoraipakkam (Chennai) for mainframe developer (Associate) role in cognizant.
I cleared all the rounds and including hr round. They made me fill a form with all my details on the same day.
Hr told me that with in 1 week i can expect the salary break up and once i accept it i will get the offer letter with in 2 weeks.
Its been 2 weeks now and i have not received any reply from them.

Hi,
I am not able to enrol my policy in LIC corporate portal.
After filling all the details it show me error as in attached screen shot.
