Cognizant — pf account number

Hi, this is venkatesh jami. I would like withdraw my pf amount now. Problem is i dont have idea of my pf account number even right now i dont have my payslips also. Only the proof is my employee i can share is 362786
I used to work with cognizant in the year 2012 in nanakramguda, financial district.in the department of post settlements. Please help me to get my pf account number, so that i can try for withdrawl.
At the time my tm is prasanth and tl is shireen.
