My name is Fazil, I was with Cognizant Hyderabad for 2.5 years and left the company on 12th Jan 218.

This email is to know more about my online PF + Pension transfer – Cognizant recently changed the account for all the employees [from APHYD to TNMAS]

Please find attached online submission form 13. I believe this form should be duly signed by myself and submitted to cognizant office for further processing

Can you please assist on few questions below?

· This can be sent to above address via courier?

· Should one of my friend can submit the form at the given address [in point 1]
