Cognizant Technology Solutions — Pf withdrawal still pending

I have submitted my pf forms and details to the above mentioned company for pf withdrawal procedure last 2 years back. Now it has been processed but it rejected with status as mentioned below:

Reason of rejection: form 15 g / 15h
Remarks by da/ao:member not signed in delcration of non-employment, form-15g & it pan not enclosed./oknot signed/ attested
Remarks by da/ao:member not signed in delcration of non-employment, form-15g & it pan not enclosed./ok

But i have submitted the forms which the company has provided earlier 2 years back. I am now clueless as what is going on with my pf settlement.


Please advice or help me on this issue.
