UNDER RTI ACT -2005
I am the holder of gas connection no 100191 of chavi gas agency roorkee & I need following information under RTI ACT-2005
1. My home address has been changed in DEC 2012 . making many repeated requested in writing along with the photo copy of govt. allotment letter my home address is not being changed till date . tell me the circumstances under which my home address is not being changed till date.
2 what action will be taken by you against the employee who is responsible for this negligence .
3. what is the actual weight & actual cost of the cylinder.
4. whether the cost of home delivery is included in the amount received by you by the customer .
5. Please provide the copy of the orders under which you are booking the gas after 22 days from the delivery of cylinder.

Sir ,
I have post the RTI application along with the Postal order through registered post having no. A 2779 to the information officer of CHAVI GAS AGENCY BSM CHOWK Roorkee on date 17/04/2012. But the CHAVI GAS AGENCY BSM CHOWK Roorkee denied to receive my application with the remark “ lene se mana kar diya”
The said refunded application I received on date 19/04/2012 . This is why Ihave to approach to higher authorities.
