[Resolved]  Family Mediclaim Policy — Non Receipt of Policy & Cards

Dear Sir,

I deposited cheque no. 445309 dt. 26/10/09 on ICICI Bank Ltd., Faridabad for
Rs. 17,755/- towards the renewal of Family Mediclaim Policy no. 221000 / 48
/ 08 / 06 / 00002885 which was valid upto 30.10.2009.

The cheque was deposited at your Neelam chowk branch, Faridabd on 26/10/09
and bank has cleared the payment on 30/10/2009

Earlier there was no response from the officers of this branch, now they say that the
cheque was not deposited by you for this policy,

kindly help us in getting the policy renewed with cards for our use.

Your early reply in this regard would be highly appriciated.
