I had purchased a Life Time Policy No.02529716 & I have paid premium regular for three year through ECS directly from my husband Saving A/c No.[protected] in ICICI Bank Sector -18, Noida.

I want to discontinue my above mentioned policy with immediate effect.

So you are requestred to please discontinue my policy immediately & confirm me accordingly by E.Mail.
