Mutual Funds — Non-Receipt of Statement since 2004

Sir,

1. We are the holders of the following Mutual Funds o[censored]TI, details as under:-

(a) UTI Master Plus – 200 – Kitty Sharma, Akhil Sharma – ISIN-INF189A01061
(Account No. IN[protected]
(b) UTI Master Plus – 100 – Akhil Sharma – ISIN-INF189A01061
(Account No. IN[protected]
(c) UTI Master Gain – 300 – Akhil Sharma, Kitty Sharma – ISIN-INF189A01053
(Account No. IN[protected]

(Note: All above have been demat with ICICI Demat services at Gurgaon)

2. Details as on 11 May 2004 of the above are given in the photocopies of the return forwarded to me by you.

3. Subsequently the above Demat Accounts were closed but to date we have not received the final statements for all the three Accounts at para1 above. We are therefore not aware of the status of the UTI Master Plus qty 300 and Master Gain qty 300, and the amounts of redemption thereof also not credited to our Bank Accounts.

4. As almost six years have elapsed since the closure of the above Accounts and details of the status of the Mutual Funds are not known. You are requested to forward the last Demat statements of the above three Accounts to enable us to claim the redemption amounts of the UTI Mutual Funds.

5. An early action is requested please.
