[Resolved]  Passport — status not clear

I have applied for fresh passport on 30 August 2010.
Details are as below:
Applicant's name: KAKA SINGH
File reference no.: GUW-A[protected]
Applicant's DOB:[protected]

Issue is that status displayed is not clear.
Is there any requirement of documents against application.

Below is status displayed :
" Furnish attested copy of school certificate /Birth certificate (those born after 26/01/1989, only birth certificate is applicable) in proof of your date and place of birth. In case of illiterates, an affidavit, as per annexure-A of the instructions attached to the application,may be furnishedduly attested by a magistrate or a notary public.

Furnish "Identity certificate" (AnnexureB,NOC) in original from your heda of the department if you are in service of Govt./Public undertaking/statotory body. Please attach a copy of your ID card.
