I myself further clarify and confirmed that my full name is KANTA YADAV
as the same appears in PAN CARD as well as in Bank Statement of accounts,
but name was mentioned in Educational certificates as Kanta Kumari Yadav
(i e Before Marriage)

Also I am enclosing herewith a attested photocopy of PAN CARD, Telephone
Bill, and a certificate issued by Syndicate Bank for confirming the name and
who is dependent on Sri Naresh Kumar presently working as a Manager in
Syndicate Bank, R K Puram IV, N Delhi (Their File No:A062073/Passport
received )----- This letter has already dropped in letter box placed in front
of your office on 18/02/2010.

As more than 70 days expired, I humbly request you to take up the action as
early as possible after clarification in my name as KANTA YADAV to avoid
further delay.
