With reference to the file no LKOZ28146308, the passport application form submited on 02-05-08 at Kushinagar District Pass port office .Although all verification have complited. Through Internet I came to know that the passport will be dispatched on 26-11-08.

But i did not received till February 2009. So I consulted the enquiry counter he directed me to fill up New passport application form No 1 . So i submited the above furnished form on 02-03-09 .

Therefore, I request you to kindly pay attention on the above matter and do the need full as soon as possible.
