Most humbly and respectfully I beg to say that the name written in the passport application form receipt is wrong in place of Chetan kumar gupta the name written is Chtan kumar Gupat. So, I request you to edit the name for this I am very obliged to you.

Passport office , Patna — Regarding Wrong named in Passport

I Applied for my passport some months before, My file no is A005492,
Some days before I got my passport but unfortunately my name is wrongly mentioned in the passport.
My Name is 'Shaila Tulsyan' but name mentioned in passport is 'Shaila Kumari Chirania'.
This was my name before my marriage.
So You are requested to Please guide for the process of how to rectify the name in the passport.
