Dear Sir,

Myself and my Husband applied for a fresh Passport on 01-04-11,as advised by the Office on duty I attached Annexure ‘D’ in original with my Husband’s Application and photocopy on my Application

While checking Application status on Website on 07-06-11 I noticed the objection of name difference in the document submitted. On 09-06-11 I had personally met a Sikh Office in Z Section( after standing in Queue for about 2Hrs) and handed over a fresh ORIGINAL Annexure ‘D’ and I was assured that the Passport would be dispatched in15days time but so far nothing has happened and Website still showing the same objection, I hope this time Annexure ‘D’ Original form is either been lost or misplaced

It is obvious that I have to suffer due to lapse in your office therefore again submitting the following documents

! Once again a Fresh Affidavit in Form ‘D’
2 Photocopy of my Husband Passport showing my name as Wife
3 Copy of the earlier 2 Annexure ‘D’ submitted on 01-04-11 & 09-06-11

I hope this time these documents would be put on my File and I will get my Passport soon
