Sub: - Non receipt of EPF & EPS withdrawal cheques (PF A/c No DL/19936/810)

Dear Sir,

I, the applicant, beg to inform you my employer SMEC India Pvt Ltd applied for my EPF & EPS fund withdrawal on January 2009. As I did not heard anything from you I lodged complaint under Public Grievance (Registration no CEPFO/E/2009/04723). The updated status on 12th June 2009 I got that is under process with your office.

Last 25th June 2009 I received the information from your office,
The PF A/c No.DL/19936/810 has been settled as per following details:-
PF - Rs.102759- Ch.No.788672 dated 30.4.2009
EPS - Rs.6630- Ch.No.330093 dated 30.4.2009

But those cheques were neither received by myself nor were it was cleared any bank A/c of me, if it has been sent directly to my bank A/c. I provided my Standard Chartered Bank JMD branch (A/c no [protected]) during applying to deposit my amount.

I am totally confused that when I got information on 12th June that the application is under process, subsequently I got information from your office that cheques against the amount were issued on the month of April.

Therefore I request you to do further by which I may get that assigned cheques.
