Dear Sir/Madam,

I am holding a mediclaim policy bearing policy number-411401/48/10/01495.My employee ID is 488.

Inspite of several followup,nothing is happening.The surgery took place in the month Jan 2010 & i had submitted the claim in the month of Mar2010,But the claim is yet to settle.

So kindly look into this matter & get back to me.If any help require from my side.

We have send a registered post to Mr. T radhakrishnan , Saivihar Blai Parambil Road, Toll Junction Edappally,Ernakulam- 682024 on 07.01.2010 vide Consignment numb- RLA-1521. the same is not yet received by the Consignee.

Please check and revert the status on an urgent basis
