My mother Late Smt Chohari Devi, aged 66 years, W/o Late Shri Shiv Govind, R/o 106, Bal Mukund Khand, Giri Nagar, Kalkaji, New Delhi-110019 died in Emergency Ward of Fortis Escorts hospital, Okhla, New Delhi on 21st April, 2021 at 8.20 PM.
2. In this I would like to submit that on the advice of office bears of record room of your hospital, I applied online application for death certificate on 13.05.2021 bearing application number[protected]. Now, SDMC has sent a message stating that the registration has been cancelled as registration is required to be done by the hospital.
3. It becomes pertinent to mention that I have to deposit Death certificate of my mother to various places including Govt offices for various purposes.
I, therefore, humbly request you to kindly get registration done as soon as possible so that SDMC is in position to do the needful.

In continuation of my email regarding problems faced in issuing death certificate in respect of my Mother Late Smt. Chohari Devi i would like to inform you that as per advice of Administration of Fortice Escorts Hospital, Okhla, New Delhi I have applied three times for issuing Death Certificate online on portal of SDMC but all the times my application are being rejected stating that this registration is to be done by the hospital concerned. I have tried to mention the fact that the patient was declared brought dead and enclose all necessary documents issued by your hospital but SMDC has been sending rejection letters all the time with only one reason that this registration need to be done by the hospital concerned.
I have to submit death certificate for many purposes in NCERT for taking further necessary actions.
I, therefore, request you to kindly do the needful so that further delay is avoided.
