Both me & my wife had taken both doses of covid shield vaccination on 19.03.21 & 29.04.21. Our registrations were done by the personnel on duty on 19.03.21. We have not received any confirmations about vaccination taken either in our mobile number, or any certificate. All we got are " FORTIS COVID - 19 VACCINATION CARD".
We therefore, request to solve the problem & provide us with vaccination certificates for both o[censored]s & confirmation of vaccine taken for both self& my wife in Aarogya Setu.
