HDFC Bank — I didn't get my cashback of rs. 6000

I purchased a samsung galaxy note 10+ which was in offer of 6000 casback after 120 days, its been 6 months i didn't get by cashback. I'll mention the loan number and further details below . it was under costumer durable loan scheme . please do the needfull.
