I Anupam Maiti, husband of policy holder Srabani Kuilya(Maiti), Policy No.[protected] Premium Rs. 6569.00, Date of Birth 24 Jan 1982. I paid premium by net banking on 16 Mar 17. The premium amount Rs. 6569.00 debited from State Bank of India account no [protected]. But receipt of paid amount not generated and premium not paid till to date.

Therefore it is requested that premium receipt may be generated at early date. And confirmation to be send this email address, [protected]@gmail.com/[protected]@gmail.com
