Lucknow Nagar Nigam — Regarding name transfer certificate issue.

My name is sandeep kumar singh, my residence place present in nagar nigam jone 8, jonal officer name is mr. Suraj singh,
I applied for name change in nagar nigam on date 25 feb 2017, and deposited name change fees with all the document required for name change process. One notice issued by nagar nigam for insepction on date 25 feb 2017. Both document copy i have attached as an attachment.
After one month i got confirmation that name has been changed but not issued name change certificate (Yellow card). I am visiting nagar nigam jjone 8 office from past 2 month approx but no body is listening, clerk which was assigned for my case mr. Ayub got captured in fraud and suspended from his duty. So now they are telling me from past 1 month that they don't have file and till now mr. Ayub didn't handover any file so that they can't help me because they don't have file. And they are not also telling me that till when they will issue name change certificate.
