Website: Two phases are not wonking

I have 3 phase domestic connection with customer ID [protected]. I am not getting power on two phases since 19th May Night. My House no. is B-181, New Amritsar, GT Road Amritsar-143001 under Mall Mandi Sub Division.
The power failure is since midnight of 19th May 2016. The transfrmers are not being well maintained and the customers face difficulty.
