Reliance Communications — complaint about epf, reliance employee made a mistake.

Sir my name nahikoti pooja previously i am employee in reliance trends, employee no 60351782 upparpally store hyderabad,
I wanted withdraw pf but getting error that is showing name mismatch between aadhar and uan account, in my aadhar my name is nahikoti pooja[protected] but in uan account[protected] showing name pooja nahikoti i can send request for name change in uan for reliance majestic office in hyderabad but pf details checking employee not changing my name in uan. Same mistake in bank account [protected] (Sbi) in my account nahikoti pooja but pf account showing pooja nahikoti. Please change my name correctly put everything in nahikoti pooja... My health is not good please update my name in uan same aadhar... This is my pf account number mh/va/117678/253903.. Please change...
