Sandeep Tiwari — high school marksheet name correction

Sir..., i am sandeep tiwari. I have passed my matric in 2012 from u. P. Board. My father name is ramvilash tripathi. My d. O. B. Is 25/10/1997. I have passed my highschool examination from hcpumv lodhan shivpur varanasi u. P. I want to change only my title from tiwari to tripathi as my father name title & i want to erase let from my father name starting. My roll no. Is 3638707. Please sir help me... Thank you...
