My customer id[protected] and pol no 057587. I had planned to be admitted in Fortis
Malar Hospital in Chennai to operate Carotid and Vertebral arteries. On 9th aug 2016
I became unconscious for one minute while I was in the church. Followed by
examinations and investigations at nearby Apollo Hospitals by Dr. Balakumar.S.
It was diagnosed as both the carotid and both vertebral arteries having significant
stenosis. As the case to be urgently treated I found to be admitted in Fortis on
good prospects.
When the hospital approached star health for cash less treatment the CLAIM NO
0159813 been rejected for cashless as the disease can be be chronic.
As a marine engineer I am sailing in merchant navy from 1990 continuously with
out failed any time on medical grounds. My last assignment completed and I signed
off from vessel OSL GLORY on 30th june 2016. Before joining every ship a strict
medical check up as per ILO CONVENTIONS being followed. The DG SHIPPING
GOVT.OF INDIA Approved clinics issued medical certificate copies attached.
Last five years continuous sailing details mentioned in CONTINUOUS
DISCHARGE CERTIFICATE CUM SEAFARER'S IDENTITY DOCUMENT
are attached. Also service letter from companies for the last two years attached.
The investigation reports based on MRI and DOPPLER test copies attached.
Rejecting cashless treatment under these circumstances will be wrong precedent
for the more than 5 lakhs sea going officers under the association all over INDIA.
Hence I request you kindly go through the details and reprocess my case for
eligibility of cashless treatment at Fortis Malar Hospital preventing delay in this
serious case.

Dear Naresh,
Thank you for your initiation. The doctors have planned to operate one artery at a time and the next will be operated in 2 or 3 weeks after. So all the four arteries will be treated in two months time. Delaying the process may lead to stroke or paralytic.
I am expecting the insurer co-operation in this very much. The hospital has to plan for hard wares like stents well in advance.
Hence I kindly request you to guide me to solve this issue asap.
