Sir/mam, My medical policy no P/111111/01/2017/009843 family Health optima Insurance policy in the name of D Ramkumar I have forwarded the Master Health Check Up claim on 27/04/18 amount of Rs.2180/- as per your instructions I have made the check up on 29/03/18 but so for not received any amount at your end. I have started in the year 2012. One fine morning your annanagar Branch staff call me and inform that this type of policy not eligible for this claim, same I have informed that your staff only inform me for which I have made the Health Check Up so for not give any reply at your end.
