Unit Trust Of India Mutual Fund [Utimf] — Rajlakshmi Unit Plan (II) - Benefit not availed

My grandfather had bought this plan under my name, Aditi Suryakant Bairalu, dated 19/07/1996. This matured on 19/07/2016. I have not received any status about this. Please help me with this and guide me.
