Subject : Request for Resolving Insurance Fraud Case: Policy No:[protected]/[protected]
Dear Sir,
I hope this email finds you well. I am writing to seek your intervention in a distressing matter involving a fraudulent insurance case that has affected my family, specifically my father, Mr. Ashok Kumar Chopra, who is a senior citizen at the age of 75 years.
I would like to provide you with a detailed account of the sequence of events leading to this unfortunate situation:
1. In December 2021, my father was approached by an individual named Ms. Prachi (Contact no. [protected]) representing Bharti Axa Insurance. She presented what she claimed to be a special insurance plan for senior citizens. She introduced another person named Rahul (Contact no. [protected]) who was supposed to provide detailed information about the plan. My father agreed to this plan, which required an annual payment of Rs. 3 lakh for three years, with a promised return of Rs. 9 lakh plus 10% interest at the end of three years. All necessary documents were signed at our house, with the understanding that we would make manual premium payments.
2. In September 2022, Rahul approached my father again, this time offering a one-time plan requiring a payment of Rs. 5 lakh, with a promised maturity value after three years and a 10% return. This policy plan was to be made in my father’s name, as it was specifically for senior citizens but Rahul insisted on submitting nominees KYC along with bank details .All documents for this plan were signed at Bharti Axa Insurance’s office in Sector 34, Chandigarh.
3. In December 2022, my father received a notification of a Rs. 3 lakh debit from his bank account. He immediately contacted Rahul, who assured him that this was an error and promised to rectify the situation to ensure manual premium payments.
4. The situation came to a head on, when I received notification on September 5, 2023 of Rs. 295 debits from my bank account due to ECS/NACH dishonor. Upon investigation, I discovered that an insurance policy for Rs. 5 lakh in my name (Ankit Chopra) had been issued without my knowledge or consent (documents signed were with perception of being nominee). It became clear that we had fallen victim to a fraudulent scheme. When contacted, both Rahul and Prachi were unreachable, and it was later revealed that there were no such individuals employed by Bharti Axa Insurance.
To add to the complexity, the insurance plans we were issued were entirely different from what was initially presented to us. My father, now in a state of shock and distress, had invested his life savings into these policies, amounting to a total of Rs. 11 lakh in premiums paid.
As a middle-class family, this situation has put us in a dire financial predicament, and it is impacting my father’s health. Our son is currently in the 9th grade, and we had meticulously planned our funds for his future education and other essential needs.
We kindly request your intervention and assistance in this matter to help us recover the principal amount invested, as was initially promised to us by the individuals involved in this fraudulent scheme. We believe that your expertise and authority can play a pivotal role in bringing those responsible to justice and ensuring that our family does not suffer further financial and emotional distress.
Looking forward for your prompt action
Yours Sincerely,
Ankit Chopra
