Dear Sir,
As per the above mentioned circular of RBI, I had submitted my father's (91 Years Old) Life Certificate on 2nd Nov, 2020, through mail to SBI- Ottapalam Branch and copy marked to CPPC Trivandrum. The said format was signed by my father, but yesterday(3rd Nov, 2020), I got the mail from SBI Ottapalam, saying that the forms are to be attested by Gazetted Officer.
I have attached the formats for your kind perusal.The authorization portion is completely striken out in the said format.

Would appreciate if you could clarify the same ASAP.
