Chaudhary Charan Singh University [CCSU] — name change in marksheet

Sir,
Myself supriya from baghpat.
I have been completed bsc (Computer science) from dn college meerut.
My complain is that my name in 10th/12th marksheet is 'km supriya' but in graduation marksheet is supriya, so this cofusive situation can creating problems at govt job document verification stage.

My request is that please suggest me the way through which i can change my name in graduation marksheet.

Mane apne sister ka form fill kiya tha us me father name mistake ho gya hai karna tha (NOORDEEN ALI) our ho gya (NOODEEN ALI).
SIR karpeya is form ko shahi karne ki karpa kare. form No. T no. 9308.

Sir by mistake meri date of birth examination form mai wrongly enter ho gayi please correction kar dijiye (bcom private (u.g)1st year)
