Cognizant Technology Solutions — not able to apply for new hdfc food card

My current hdfc food card is not working, when ever i do a swipe, card is getting declined, so i planned for a new one and tried to submit for a new one in one cognizant portal (https://leads.hdfcbank.com/applications/webforms/apply/foodplus.asp) where i am getting a message like "user id has been already registered", can please help me how to get a new hdfc food card and transfer the amount from old card to new one.
