ICICI Bank India — ATM WITHDRAW MONEY NOT RECEIVED BUT DEBITED FROM BANK ACCOUNT

I tried to withdraw 5000/- from my SBI a/c [protected] OF SBI BRANCH CGO COMPLEX LODHI ESTATE NEW DELHI through icic bank atm "indian spinal injuries centre" ATM ID S1VD7210 at 15:26 hrs on 05/07/12, BUT said amount debitted from my a/c and TRANSACTIONED DECLINED vide TXN NO 5290 , RESPONSE CODE 070.

IT IS ALREADY MORE THAN ONE MONTH HAS BEEN ELAPSED AFTER TRANSACTION, THEREFORE, PLEASE REFUND/CREDIT THE AMOUNT IMDTLY
