I would like to bring to your kind notice, that my name was entered by surname only, in my correspondence and academic records I have entered my name with surname and my father name i.e., A N Shiva Shekar the same proof was produced at the time of new gas booking connection in the year 1993 but in your records you have entered as, A. Shiva Shekar. So I request you to change my name and address.
