I have paid Rs.250 in State Bank of India and got cash voucher. I could not find application form in sbi website .How to get it ?

HOW CAN I GET ADMIT CARD?

BM, SBI Satgram is always non cooperate with Raniganj BDO Office.BM is very arrogance & customer service is very poor.Pl look into the matter.not receiving OTP
