Sir,

Sub: Double entries for a single withdrawal from ATM on 14/12/2009 - request for
Restoration of erroneous debit - reg
Ref: Savings Bank Account No.[protected]

I have a Savings Bank Account No.[protected](SBT JEROME NAGAR-MAIN BRANCH) with you for credit of my Pension.
On 14/12/2009 at 18:19:hrs. I have withdrawn from the ATM installed at your Branch, a sum of Rs.6,000/- using my State Bank ATM card .But today I got my passbook updated and found that instead of Rs.6,000/- withdrawn on 14/12/2009 two entries for this single withdrawal ( the same amount - double entries) are seen recorded and debited to my account erroneously.

I there fore request that this mistake may kindly be verified and the irregular entry on 14/12/2009 may kindly be cancelled and necessary credit may be given to me with effect
from 14/12/2009 itself for the sum of Rs.6,000/-
