HCL Technologies — get pf transferred from hcl to current employer pf account

I left HCL ISD (Infrastructure Services Division) in October 2013. In order to get my PF transferred to my PF account with the current employer, I am required to fill in the following details in Form 13 Part B:

1.Name and address of previous establishment (Point 2 in Part B)
2. (PF Account is held by: (Name of EPF Office/ PF Trust) (Point 3 in Part B)

From my salary slip, I am able to locate the following details:

Location (CWL): Noida-SEZ, Infra-2, U-6, Flr-5th
Department: INFRA DEL-US-EC2-N1
The HCL Division name that appears at top of my salary slip is: HCL Tech. Ltd. IOMC.

Please provide me the information required in points 1 and 2 above, so that I can request PF transfer using Form 13.

Also, please let me know if I can submit hard copy of Form 13 to my current employer.
