HCL Technologies — Pf incorrect information (name and d. o. b. mismatch)

I am ex-hcl employee with following details:

Name - hardeep singh

Hcl pf no - gn/ggn/5572/000/319129
Date of joining - 16-mar-2015
Last working day - 23-may-2016

When i try to initiate pf transfer from epf website, it is showing wrong member name (Soumya ranjan satapathy) and wrong date of joining. Due to this discrepancy i am not able to proceed with my pf transfer.

I have opened up a ssd ticket (#152249) with ex-hcl portal, but no reply for over one month now.
