My self Mohammed Riyaz having Kokilaben Hospital Patient ID No KH[protected] who had an appointment session with Dr Vaibhav Daga (Sports Physio) on the 3rd Jan 2024 @ 12:00 pm having been referred to you under the guidance & observation of Dr Vishal Peshattiwar (Orthopedic Spine specialist).
After doctors kind observation and recommendation to visit the Gym very same time in the basement for FMS Test under guidance of Mr Faheem and get back to him. I had visited the GYM area to Mr Faheem but he outrighly rejected to see due to reasons known best to him. He then called me on the 4th of Jan 2024 i.e Thursday at 9:00 am and told me Miss preeti will take the test. I followed all his instructions and was told that after payment I shall get the FMS report on the very same day in the form of PDF which I wanted to discuss with Dr Vaibhav Daga after tkaing his due appointment. However, ever since that day Mr Faheem intentionally kept delaying sending the report and also showed 0% interest in giving me timing for the sessions despite all my best effort. Now, until today 1:00 pm he has not yet send the report and when I complained the sports physio department reception counter about it and they connected me to Mr Faheem, the manner in which he has spoken to me is completely un acceptable to me which as a patient and customer of such reputed Hospital and who has made all the payment. It is also against the Kokilaben Hospital principle of Patient & Family Rights And Responsibilities Despite, all this fact his behavior on call was very rude and pretending as if he is doing me favour by sending the report.
I never in my wildest dreams that a Hospital with such high caliber, reputation & experienced professional doctors would have a person in their team who neither has any work ethics and also treats a patient like a roadside Municipal hospital person. I completely condemn this act of behavior by Mr Faheem and would request the authorities to please take punitive action and serious against Mr Faheem to prevent such incident occurring with other patients in future. This episode has now left me in a complete shock and a bad perception in my mind and I hope no other patient goes through the same ordeal as me. I have been suffering form back pain since last 12 months which was not just overlooked by Mr Faheem but also his behavior was least concerned as if he was not interested in doing his job.
I request for urgent action in this matter. .
