Sir,
1. I am the holder of LIC policy No.[protected], due for maturity on 28 Sep 08.
2. During my last visit to Ambala Branch of LIC of India, in
Jul 08, I was advised by the dealing staff, to send them the following documents, for issue of a post dated cheque, for payment of my maturity claim :-
(a) Original copy of my LIC Policy
(b) Form for claiming maturity benefits, duly filled and
signed
3. I have already sent the above mentioned documents, under my letter No. 641206/JPS dated 04 Aug 08. through registered post, on 04 Aug 08. Photocopies of the receipt obtained from the Axis Bank, showing payment of the premium for the quarter ending Mar 08 and the receipt received from your Mumbai branch, showing payment of the premium for the quarter ending Jun 08, were also sent to substantiate extra recovery of the premium amounting Rs 135.90.
4. have not yet got the cheques for maturity claim and refund of extra recovery, in spite of repeated requests through emails. I am an Army person proceeding on retirement with next two months hence am in dire need of money.

Dated: 17th Sep, 07 through registered AD.
One more reminded has been sent to LIC dated 22nd Feb, 08
but till date no response/payment has been received from LIC.
Kindly arrange to send the payment along with interest for delayed payment.
