State Bank Of India [SBI] — Very poor services & poor communication from staffs

I filled fixed deposit form on the name of mother mrs. Jharna banik (Sr. Citizen) as per sbi guidelines but till date it not prepared. Now after continuous follow-up after 07 days passed they informing that some problems in form whereas they checked the form during submit.

My filled date of fd form is on dated:[protected]. Also neither they communicate by phone about the matter/ issue.
