State Bank of India — Transaction failed but amount deducted

I have done a transaction on 19 /04/17 at 8:13 AM through UPI. The transaction failed but My account has been debited.

Please do something. As I have already contacted to several offices like call centre, Bank branch but no one is resoving the issue.

Please help me to sortout the issue.
