State Bank of Patiala — Late account renewable

I m using a cc limit account. Last year i complete all documentation for cc limit account on time. Because of staff mistake late renewal charges cut from my account every monthly. Even not inform to me that account is not renew. I m very dissapointed with bank services.
Please take action as soon as possible.
