Uttar Pradesh Technical University — Name correction in MCA marksheet & degree

Sir, I have completed my MCA in 2005. I have observed that name in the marksheet & degree does not have my last name. Due to which at the time of document verification questions are raised. I request you to kindly correct my name in the marksheet and degree and send me across.
