I have orderd a shoe from amazon of Rs/- 499 but due to some personal issues i've cancelled my order but i haven't got my refund money yet.. I had a conversation with a lady of complaints center i told her he can say me you get a refund after 4 5 days and i not get my refunds my details than she asked me to fill this form..
So please refund my money as soon as possible.

I have payed the sum of Rs.3161/- at restaurant, money has been deducted from my account but has not been received by the restaurant. and they have given the cancelled receipt.
kindly arrange for refund ASAP.
